The parking division of one’s local municipality is responsible to arrange a parking space for the disabled near their residence and/or workplace. For each locale in Israel, the local authority should be contacted for instructions on how to submit a request. The following are instructions adapted from the Jerusalem Municipality website:

After submitting contact details including name, residential address and/or place of work, the applicant will be sent a request form by mail detailing a list of accompanying documents that should be attached to the application. Forms can also be filled and submitted on the Jerusalem municipality website, for those who can scan the necessary supporting documents.
Residents over 65 with a handicapped parking placard, but without an assigned disability percentage should submit a medical opinion from a specialist indicating the mobility disability.
All other applications will be assessed based on the percentage of disability as determined by a statutory committee such as from the Misrad Habriut, Bituach Leumi, or the Ministry of Defense:
Those with mobility disability percentage of 80% or greater are eligible to reserved parking near the house and at the workplace. If they do not drive themselves they are only entitled to a parking space near the house.
Those with a disability percentage of 60% or greater and have limited mobility are eligible to reserved parking near the house only. If they do not drive they will be entitled to parking at the residence only from 70% disability.
Exclusions and Limitations
Those who live in a home for the disabled, or any institution are not eligible for a reserved parking space.
Those who have a private parking space at home or work are not eligible for a reserved parking space.
Reserved parking near work will be limited by days and hours worked.
